PG&E DATA REQUEST LETTER


Instructions:

1) Print out and photocopy the PG&E Data Request Letter onto your jurisdiction's letterhead.
2) Type in the date above the address block.
3) Type the legal name of your jurisdiction or special district on the blank lines of the letter. There are two blank lines on the first page and one blank line on the second page.
4) Type in the name and title of the individual who will sign the letter. (Staff may sign as long as their name and title are provided.)
5) Have the letter signed.
6) Print out and complete page 3 of the Data Request by entering the service address, account number, and meter number of every account you would to include. This information is obtainable from your PG&E bills.
7) Attach page 3 to the original signed letter, make a copy for your jurisdiction, and mail all three pages to ABAG with your Statement of Interest.

Please return the completed PG&E Data Request Letter to ABAG with your Statement of Interest.
